  1. Definitions of some terms for understanding the points of agreement:
  2. Institute/ company - consider as Medreliv as proprietary firm.
  3. Address - 2537 C Ward Shaniwar peth kolhapur
  4. Area of working - A tech platform works anywhere it is needed.
  5. Customer - Any person who gets into agreement with Medreliv & ready to pay the service charges for the services he has received & the patient self who is mentally & physically able to get into the agreement with Medreliv will be considered as Customer.
  6. Duration of services - As per requirement of customer.
  7. Reliver/care taker/care person/care givers/Associate - Any person who has completed 18 years of age, with good physical & mental health, qualified & experienced who will be in agreement with the company & ready to offer his services to the customer.
  8. Eligibility for Reliver - Any person who completed 18 years of age, either qualified or experienced or both, hold good knowledge about patient care.
  9. Role of Reliver –
  10. 1. He/she should follow advice of doctor of concerned patient.
  11. 2. He/she follow the normal code of conduct.
  12. 3. He/She follow the discussed schedule of services.
  13. Renumeration –
  14. 1.Payment will be transferred online every fortnightly by the company. Reliver will not ask for any other compensation directly to the Patient.
  15. 2. Not entitled for any government benefits.
  16. 3. No any payment in advance.
  17. Code of Conduct –
  18. A. Reliver will follow the professional rules & regulations. Will not misbehave with the patient & any of family members.
  19. B. He/she has right to get total information about the patient. Reliver will offer his services under medical supervision. He/ she morally & legally responsible for his services
  20. C. Reliver is solely responsible about medicine administration. Any irregularities found Reliver should inform to the company.
  21. D. Reliver will not enter in any area of the house unless it is needed & with the permission of the customer.
  22. E. Reliver should follow the schedule which is expected. If any problem arises Reliver should inform to the company before 24 hrs.
  23. F. Reliver should not have any bad habits & will be responsible for his / her outfit.
  24. G. Reliver will not perform any personal task besides the tasks required for patient. If happened so Reliver should inform to the company.
  25. H. If any Reliver will not following the code of conduct or misusing the rights, passing on any misleading information & misbehaving with the patient & any of his family members, his account will be permanently removed & strict action will be taken by the company.
